chore: untrack the local codegraph index folder

What

.codegraph/ is a per-machine CodeGraph index. Drop the tracked .codegraph/.gitignore and ignore the whole folder from the root .gitignore instead of .codegraph/*.

Risk

None: no app code. Local indexes are untouched.
